Instructions for Making a Request

Only government departments accept online access to information requests through this website. To submit requests to other public bodies such as agencies, healthcare and education bodies, and municipalities, please follow the instructions provided on the ATIPP Office website.

When completing the ATIPP request form, please remember the following:

  • Make only one request on each application form.
  • Describe the records or information you are seeking in as much detail as possible.
  • Keep a copy of the confirmation number you receive when you submit a request for your records.
  • Please note that under the authority of section 25 of the Access to Information and Protection of Privacy Act (ATIPPA) you may be asked to pay certain additional costs as prescribed by the Minister responsible for ATIPPA, before gaining access to records.
  • Please note that if the department does not respond within 20 business days of receipt of this application, you may ask for a review by the Information and Privacy Commissioner, under authority of section 42 of ATIPPA, or you may appeal to the Supreme Court Trial Division.
  • The ATIPP Act may be viewed in its entirety at http://www.assembly.nl.ca/legislation/sr/statutes/a01-2.htm.

Records responsive to general access will be published following a 72-hour period after the response is sent electronically to the applicant, or five days in the case where records are mailed to the applicant. It is the goal to have the responsive records posted to the Completed Access to Information Request's website within one business day following the applicable period of time. Please note that requests for personal information will not be posted online.

Personal information on this form is being collected under the authority of s.61(1)(c) of the Access to Information and Protection of Privacy Act and will be used only to respond to this request. Inquiries about the use and protection of this personal information should be directed to the Access and Privacy Coordinator of the department to which the application is sent.
